East Palo ALto Community Health Network
The East Palo Alto Community Health Network (CHN) brings together community-based organizations, healthcare providers, and residents to build a healthier East Palo Alto, together.
Convened by ‘Anamatangi Polynesian Voices through a Get Healthy San Mateo grant, the CHN is a community-centered space where lived experience leads the way. We center the voices and needs of youth, families, and elders to work across organizations by sharing information, strengthening relationships, and taking collective action.

Making existing services easier for residents and organizations to find, understand, and access.
The annual Mama Dee Day Pacific Islander Festival in August 2026 carried the theme ““Healthy Lifestyles, Healthy Families.”
The event brought together community-based organizations, healthcare providers, cultural performers, musicians, merchants, and food vendors.

The CHN Youth Advisory Group brings young people into the conversations shaping health, wellness, and opportunity in East Palo Alto and East Menlo Park.
You'll sit at the table with community organizations, leaders, and professionals, and help decide what the network works on next.
Youth ages 14–24 who live, work, or go to school in East Palo Alto or East Menlo Park. All backgrounds welcome.
Who can apply?
Join the monthly CHN Community Meeting (Zoom and in person)
Attend Youth Advisory Group meetings between CHN meetings
Share youth perspectives and community needs
Help develop youth-focused ideas, activities, and solutions
Connect with community organizations, leaders, and professionals
